The Evolution of Gynecomastia Surgery

If we look into the history of surgical procedures for gynecomastia, it was physician and philosopher Galen who first introduced this term in the second century. He described gynecomastia as an unnatural increase in the breast fat of males. Although Galen was also aware of glandular enlargement of the male breast but he considered this a separate problem and not ‘gynecomastia’. There is no evidence from historical sources about a specific treatment by Galen for this ‘unnatural increase in breast fat’ but it was in seventh century AD that an attempt at surgical correction of gynecomastia was made. This was in the form of a reduction mammoplasty by Paulas of Aegina. He referred to the condition of enlarged breasts as an “effeminacy of men.” In his surgical works, Paulas described the details of surgical procedure for male breast reduction.

He described a surgical procedure employing a moon-shaped sub-mammary cut with provision for resection of skin in pendulous cases. Paulas also mentioned the need for secondary surgery in some cases, stressing that primary procedure may prove inadequate for severe cases.

Treatment of gynecomastia continued to evolve and improve over ages. Cosmetic and revisionist surgeons today use a multifaceted surgical approach to optimize correction of the deformity. The fat from breasts is removed with different liposuction procedures, while the glandular component is removed with direct excision. Skin is then re-draped over underlying structures and is resected in severe cases. Modern surgical techniques used for male breast correction help to minimize the recovery time and bring more effective cosmetic results than what were noticed a few decades back.
